Protocol and Guidelines for Patients Biochemical Profiles

 

There has been some confusion over exactly what is needed, when and in what order, with regard to the required paper work, testing and analysis need to develop a patients’ biochemical profile. In order to process the patients’ profile in an orderly fashion, we must receive the completed documentation as follows:

  1. Physician Confidentiality Agreement

  2. Completed Medical Questionnaire

  3. Patient Consent Form *   
     *  Must be signed by the patient and completed with a physical street address.  (UPS will not deliver to P.O. Boxes.)

  Blood Test Kits

Amino Acid 40 Plasma and Fatty Acid 40 Plasma Test

The Amino & Fatty Acids Test Kit has detailed instructions that must be strictly followed:
  • Test Kit must be ordered in advance. (3 to 5 days)
  • Test Request Form must be filled out completely.
  • Payment must accompany sample. $365. Personal check or Money Order O.K. (NO insurance.)
  • Schedule blood draw Monday through Thursday and coordinate the time around AirBorne Express' pick-up time so the processed  sample will arrive at MetaMetrix within 24 hours of being drawn.   
  • Patient must fast for 12 hours prior to draw. (Dinking water is O.K.)
  • Polar Pak (in the Kit) should be frozen over night before the blood draw.
  • The Lab may charge a phlebotomy fee. (usually about $25)
  • Sample must be immediately centrifuged and prepared following directions included with the Kit.
  • Sample and Polar Pak must be frozen prior to shipping.
  • Sample should not be sent with or packed with Dry Ice.  Sample need not arrive in a frozen state, just be frozen prior to shipping so that it is kept cool in transit.
  • Blood draw for the Cell Function Test can be done at the same time as these tests. 
Failure to follow direction will result in having to re-draw the blood sample.

Cell Function 5000 Series Test

The FIA Comprehensive Profile 5000 Test Kit has detailed instructions that must be strictly followed:
  • Test Kit must be ordered in advance. (3 to 5 days)
  • Test Request Form must be filled out completely.
  • Payment must accompany sample. $310  Personal check or Money Order O.K.
  • Schedule blood draw Monday through Thursday and coordinate the time around FEDEX's pick-up time so the processed sample will arrive at SpectraCell within 24 hours of being drawn.
  • No Fasting required. (O. K. if fasting for other tests)
  • The Lab may charge a phlebotomy fee. (usually $25)
  • Sample must be drawn with a 21 gauge needle and handled following directions included with the Kit.
  • Sample should not be centrifuged, refrigerated or frozen.
  • Blood draw for the Amino and Fatty Acids Tests can be done at the same time as this test. 
Failure to follow direction will result in having to re-draw the blood sample.

NOTE:  Doctors: Please be sure to place both your name and the Cochran Foundation name on the “Ordering Physician” form for both tests.  Failing to indicate CFMR will delay our analysis.

 

Visa or MasterCard information: 

Patients may use either their Visa or MasterCard to pay for all Cochran Foundation (CFMR) services and products offered on the web site.  (Sorry, these are the only cards we currently process.)  All credit card information is handled electronically on a secure server and will only be accessed with patient’s consent and authorization.  Any other payment arrangements (checks, Money Orders, etc.) will be considered on a case-by-case basis and will require manual handling with an associated manual-processing fee of $25.00 for each manual order.  (We strongly recommend using a credit card to avoid the manual-processing fee, as most patients will be re-ordering on a monthly basis.)

 

Initial Analysis Fee:

A non-refundable Initial Analysis fee of $250.00 will be charged immediately when patient signs-up for the Cochran Regimen.  This fee covers the cost of opening a patient file, reviewing and analyzing all of the submitted data forms and Amino Acids, Fatty Acids, And Cell Function blood tests to develop a Biochemical Profile of the patient and prepare an appropriate biochemical prescription.

This biochemical prescription is then e-mailed to the ordering-physician for review and an authorizing signature.

Ordering your Biochemical Components:

Once the doctor reviews and authorizes the patient’s prescription, a copy of the prescription will be e-mailed to the patient. This prescription will explain which biochemical components are needed to move the patient’s present biochemical levels to an optimal Range. The biochemical components have been custom made to fit your actual biochemical condition in the effort to restore them to normal.  The patient may order their prescribed biochemicals on-line using the Cochran Foundation Website and their Visa or MasterCard charge card.  All biochemicals are produced by and will ship from a US FDA approved drug laboratory.  The Website Order Form will list the biochemicals, with a pricing schedule.  Shipping costs will be based on the form of shipping used, such as UPS next day air or UPS Ground Service.  (A special bulk rate has been worked out with UPS to lower the shipping costs.)  When the order is completed the patient may print an Invoice on their printer showing what they have ordered. Order confirmation is e-mailed back to the patient verifying when shipment has been made and will have the UPS tracking number and the date it was shipped.  (With this number the patients may track their shipments using the UPS web site.)

Without your Physician’s diagnosis, we cannot successfully analyze the patient’s data, nor can it be compiled for comparison of treatment for patients with similar conditions.

  All required forms are available and can be completed on-line.  As part of the data entry process each patient will be assigned a password that will grant them future access to their information now stored in the patient’s secured electronic file, accessible only to the patient, their physician and our medical researchers.

Again, it is important that we receive all the required forms and information, fully complete.  We are striving to make it easier for the doctor, their staff and their patient group to complete the analysis process quickly so that treatment can begin as soon as possible.

  This program is best used in the prevention of age related diseases or the treatment of biochemical deficiencies where disease specific signature profiles can verify which biochemicals are deficient.

It is important that retesting should be done every 90 days, along with constant, consistent monitoring, until the deficient biochemicals are restored and optimal ranges are reached, at which point the patient is put on a fixed regimen, with retesting every six months.

All of us have multi-genetic code that declines in essential biomarkers; these make up part of the biochemical signature of disease.  It is multi factor in that when the biomarkers are at these declined levels, which are biochemical abnormity’s, over a long period of time that the disease condition is manifest.  If you want to help your patients and get them involved here are the methods which can help and assistance them.
